1. I
agree that in the event that my child exhibits signs
of illness or injury and if the Tiny Tees Golf (“TTG”) staff is unable to contact me or
any emergency contact listed above, my child may be
taken to the nearest medical facility and given any
treatment deemed necessary by the personnel of the
medical facility.
2. I
authorize TTG to use my child’s name, video, or
photograph at any time and in any manner in
connection with its advertising, publicity, and
public relations programs.
3. I
understand that injuries can arise by accident from
the very nature of golf activities. I hereby release
and waive all rights to any claim or action against
the TTG and Pechanga Golf Club arising from
injury, loss or damage to my child and/or my child’s
property except where such injury, loss or damage is
caused by willful misconduct or gross negligence of TTG or
Pechanga Golf Club.
